What is Hospice and Who is Eligibile for Hospice Care?

 Hospice is for end-of-life care that focuses on the patient and their family rather than the illness. Hospice offers medical, emotional, social and spiritual support centered on quality of life considerations. 

  

Upon meeting the following criteria, eligibility requirements will be determined for patients.

· Based on the clinical judgement of the Primary Physician and the Hospice Medical Director, it has been certified that the patient has a limited life expectancy.

· The patient’s Primary Physician authorizes hospice care.

· The focus is now on physical, emotional and spiritual comfort rather than curative measures.

Hospice Level of Care

Routine Care

Regular scheduled care with the patient and licensed nurse 

Continuous Care / Crisis Care

Care provided during periods of crisis, requiring clinical approval by the Medical Director in collaboration with Nursing Team as defined by Medicare with a minimum of 8 hours and may extend up to 24 hours per day of continuous nursing care 

Respite Care

Relief type care, approved by Medicare for no more than 5 days in a skilled nursing facility that provides 24 hour nursing. To give family respite. (Medicare covers 5 days or less) 

General In-Patient Care

Hospital based care for those who are unstable and cannot be cared for or transported outside of a hospital base setting. (Maximum of 5 days per Medicare)

Who Pays for Hospice Care?

Most patients choose to be in the comfort of their own home during their end-of-life care. Loved ones can stay at the patient's bedside anytime, and for as long as they want, this way, they are more involved with the care of their loved one. For some patients that have LTC insurance, their insurance covers/pays for a private caregiver to help the family with the patient's day-to-day care.

Amor Hospice Care is contracted with multiple facilities in the area where we are able to deliver our services during the time when family requests for respite care or if the patient will require to be in a general in-patient care for a maximum of five (5) days for both levels of care per Medicare.

Private carehomes/group homes are also one type of residence that a patient can stay at and for hospice to deliver service. This is usually private pay, or if the patient has a long-term care insurance, their LTC insurance will cover monthly expense and might require co-pays for some policies. This is a small home setting with a group of at least 4 to 10 patients in one home with caregivers on-duty 24/7. Most families prefer to place their loved ones here because patients get more care and attention due to low census.

A lot of seniors start to reside at ALFs because they can feel that they are still independent for having their own apartment with caregivers on-duty whenever they need help, resident doctor and nurses that can check on them when they have minor health issues. This type of facility provides hotel-like living, recreational activities and also help them transition to the next appropriate level of care when patient starts to show symptoms of decline.

Admission Process

Discuss Hospice with your primary care physician

When your doctor recommends hospice care, you will be given options of hospice care program/service and options for hospice care providers. Once you or your family have decided of which hospice provider you want to walk this journey with, you just need to notify your PCP and provide the information, and they should be able to send the order for hospice, clinicals, and all necessary labs for justification. Once hospice receives the order, the hospice team will make all necessary arrangements for admission.

What if I don't have a PCP or I can no longer bring my loved one to his/her PCP?

For some patients who does not have PCP, has not seen their PCP in a long time, or can no longer ambulate to go to their PCP, you or your family can always elect for self-referral, meaning you are electing hospice at your own will. Our Nurse Practitioners are always available to visit patients at home to do an evaluation for hospice and refer the patient to hospice care if patient is appropriate or meets the qualifications for hospice care. Once the hospice receives the order for hospice from the hospice's NP, hospice will make all necessary arrangements for admission.
